Apply for a Passport at the Euclid Public Library

The Euclid Public Library is a designated Passport Acceptance Facility for the U.S. Department of State.  EPL currently offers Passport processing to travelers. Individual processing is based on a first-come, first-serve basis. All travelers, especially families of four or more, are encouraged to call (216) 261-5300 ext. 5189 prior to coming to the library to determine the approximate wait time for quicker service.

*The Library is closed on Sunday May - October

The passport application process will take from 20-60 minutes per applicant. To ensure there is enough time to complete the process, please arrive at least an hour and a half before the library closes. More time may be needed for cases involving citizenship, parental guardianship, or last-minute travel.

If a passport acceptance agent believes the process cannot be completed before the Library’s closing, you may be requested to return at another time.

If you need your passport in two weeks or less, please call the National Passport Information Center (1-877-487-2778) to make an appointment at a local Passport Center to get your passport processed in time.

 

Passport Application Services

Applying for a passport for the first time? Here’s what you will need:

  • Passport application. You will need a completed, unsigned, DS-11 New Passport Application form. Complete forms using black ink only. Do not sign your application ahead of time, as it must be signed in the presence of an acceptance agent. Blank application forms are available at the Euclid Public Library or can be found online.
  • Proof of Citizenship Click here for information on acceptable documents to demonstrate United States citizenship.
  • Current Photo Identification Click here for information on acceptable forms of photo identification.
  • One passport photo Euclid Public Library can take a  2” x 2” passport photo for you.

 

Fees

Application Fees and Execution Fees are paid separately. 
Execution fees are payable by cash, check or money order to Euclid Public Library. Starter checks will not be accepted. The Execution fees are nonrefundable. 
Application Fees are payable by check or money order to the U.S. Department of State. Multiple checks or money orders may be required for multiple applicants. Starter checks, credit, and debit cards are not accepted. Please click here for a complete listing of fees.

Additional documentation is required for minors under the age of 16. In some cases, children ages 15 and under need both parents to be present. However, there are some exceptions. Click here for more details for a passport for a child under 16. Teens between the ages of 16-17 should have at least one parent present at the time of application.
Please note: The U.S. Department of State has the sole responsibility for examining applications and issuing passports.

 

RENEWING YOUR PASSPORT

If you are renewing an expired adult passport issued within the last 15 years, you will have to apply by mail. Renewal forms are available online through the State Department website, travel.state.gov and at Euclid Public Library.
